Creatine benefits beyond muscle: what the latest research says about brain health, sleep and mood

Creatine has long been known as one of the most researched supplements for strength, power and muscle performance. But new research is shifting the conversation. Scientists are now looking at creatine for something far beyond the gym: brain energy.

Creatine helps the body recycle ATP, the energy currency used by both muscles and the brain. While around 95% of creatine is stored in skeletal muscle, a smaller amount is also present in the brain, where it supports energy production and cognitive function.

A 2025 review published in the Journal of Psychiatry and Brain Science found that creatine monohydrate may help increase brain creatine stores and support cognitive function, especially when the brain is under metabolic stress - such as sleep deprivation, hypoxia, mental fatigue or traumatic brain injury. However, the review also made an important point: optimal doses for brain benefits are still unknown.

Creatine and sleep deprivation

One of the most interesting areas of research is sleep deprivation. A 2026 study found that a single dose of creatine monohydrate at 0.2 g/kg helped reduce the decline in cognitive performance during 21 hours of sleep deprivation. The improvements were seen in logic, numerical tasks, language processing speed and reaction-time testing, with some results showing up to a 12% improvement.

This does not mean creatine replaces sleep. It means creatine may help the brain cope better when sleep is temporarily restricted.

Creatine and mood

Creatine is also being explored for depression and anxiety. A major 2023 narrative review reported that creatine shows promise for depression and anxiety symptoms, although more clinical trials are needed before firm conclusions can be made.

The theory is that some mood disorders may involve impaired brain energy metabolism, and creatine may help support that energy system. This is promising, but it should not be treated as a replacement for medical care, medication or therapy.

Creatine and concussion recovery

Creatine may also have a role in traumatic brain injury and concussion recovery. The Australian Institute of Sport notes that creatine may support cognitive processing and potentially reduce damage or improve recovery from mild traumatic brain injury.

However, a 2025 information paper on creatine and TBI stressed that large, multi-site randomised trials are still needed to confirm effectiveness, optimal dose and duration.

Is creatine safe?

Creatine monohydrate is widely considered one of the most researched and effective supplements. The Australian Institute of Sport lists creatine as a Group A supplement, meaning it has strong scientific support for specific uses in sport.

That said, people with kidney disease, those on medications, pregnant or breastfeeding women, and anyone considering high-dose creatine should speak with a healthcare professional first.

Creatine is not just a muscle supplement anymore. The research suggests it may also support brain energy, cognitive performance during sleep deprivation, mood and concussion recovery. 

For most people, creatine monohydrate remains the best-studied form and 3-5 g per day is commonly used.
